Commonly Used Forms
Event Support Request
This form is used whenever you are reserving space on campus that requires a special setup. (i.e. requires audiovisual needs, tables, chairs, off-hours programs, etc.) This "catch-all" form has information on it that will be used by several offices on campus, including Student Activities, Facilities, Instructional Resources, and University Police. It is important that this form be filled out 2 weeks prior to your event. This will ensure that there is sufficient time to accommodate your requests to make your event as successful as possible.
If you are reserving a room on campus, you must have it confirmed by the office responsible for the area.
If you are reserving contact:
- Classrooms, Kunsela Auditorium, Kunsela & Donovan lobbies - Registrar ext. 7265
- Aerobic room, racquetball courts, gym, tennis courts - Athletics ext. 7520
When this form is completed, it should be sent to the Office of Student Activities for the final signature.

Purchase Voucher
This form is used to access funds from your Student Association account. Vendor /Payee refers to the person/group/organization that is getting paid. Your form needs to be signed by your treasurer and advisor before it is dropped off to the Student Association office for a signature. Please allow 10 days for checks to be processed, so plan accordingly. Proper documentation, (receipts, contract, etc.) must accompany the purchase voucher.
Printing Request
This form is used when utilizing the campus print shop. All printed material must adhere to the SUNYIT posting policy. Your account number may be obtained from either the Student Association Office or Student Activities Office.

CAB Event Proposal
This form is used to request funds from the Campus Activity Board (CAB). Each year, CAB sets aside a pool of funds to be used to support club events. This form should be filled out completely and submitted in person at a CAB meeting. CAB meets every Tuesday at noon in the ‘Cats’ Den.

Event Checklist
This form is to assist you in organizing your event. It helps you make sure that all the ‘little things’ are taken care of. Make sure that you follow up by evaluating your event once it is completed. It is a good idea to keep this checklist with all of your other paperwork for each event with your club secretary. Your club will appreciate your accurate record keeping down the road.

Intent to Sell
This form is used by clubs wishing to hold fundraisers on campus. All fundraisers that use an outside vendor must be approved and signed off by the bookstore. This form ensures that the business coming to campus is in good standing, and also enables us to contact the owner if necessary.
