catalog publications and official curriculum files. Please
complete all
sections and route from sponsor, to school or department
committee, to dean,
to Curriculum Committee (20 copies of the completed form with all
appropriate
signatures to the chairperson). The Vice President for Academic
Affairs
will initiate final action upon receipt of signed original from
