Mandatory Health Requirements
New York State Public Health Law mandates specific health requirements to attend college. Mandatory health requirements MUST BE PROVIDED BY AUGUST 15 (fall semester) AND JANUARY 15 (spring semester) to the Health & Wellness Center (Campus Center, Room 217) or withdrawal from class will occur. Mail to: SUNYIT Health & Wellness Center, P.O. Box 3050, Utica, NY 13504-3050, Fax to: 315-792-7371 or call 315-792-7172 for more information.
Upon receipt of your tuition deposit, you will receive by mail specific information and forms regarding the mandatory health requirements. You may also obtain the mandatory health requirements and forms at www.sunyit.edu/health_wellness.
- Measles, Mumps & rubella (MMR) NY State law requires all students registered for (6) or more credits provide proof of immunity to MMR.
- Meningitis NY State law requires all students on or off campus, regardless of credits, provide proof of either having the meningococcal vaccine or acknowledged receiving information about meningococcal disease and vaccination including availability and cost. The meningitis disease information along with the Meningitis Information Response Form are in your acceptance packet. The mandatory Information Response Form must be completed and returned in lieu of vaccine documentation by a health care provider.
- Health History and Physical Examination All students registered for (12) or more credits must submit a health history and physical examination completed within the past two years by either a physician, nurse practitioner or physician assistant. You may use the Examination Form provided or a health history and physical exam form used by your health care provider.
- Health Insurance You will be enrolled and billed for SUNYIT's designated health insurance UNLESS you complete the electronic waiver EACH SEMESTER, PRIOR to ATTENDANCE. Students with (12) or more credits are required to have current health insurance coverage. If the student has other health insurance (under a parent's policy, employer, etc.) SUNYIT's designated health insurance may be waived on the web after you have a pin number, which new students receive at their initial registration.
Register with the Counseling and Special Programs Office
Students with disabilities requiring reasonable accommodations must register with the Counseling and Special Programs Office and provide written documentation to support the request for reasonable accommodations.
Confirm Attendance
Students are not officially enrolled until they confirm their schedule with the Bursar’s Office. This can be done online if your financial aid is in place at SUNYIT campus intranet or by simply returning your signed billing statement with payment or financial aid deferral to the Bursar’s Office. This must be done by the payment due date. Class schedules and/or housing assignments are not guaranteed
