Your Rights and Responsibilities at the Health and Wellness Center
Effective health care includes certain rights and responsibilities and requires a special relationship between the student, health care provider and the Health and Wellness Center. These rights and responsibilities will contribute to more effective care and greater satisfaction for you, the student. The student is ultimately responsible for his/her own well-being. The staff at the Health and Wellness Center will assist the student to reach his/her ultimate well-being.
You have a right to:
- Receive accessible, quality medical services.
- Receive quality health education and counseling services.
- Confidentiality of information regarding your care.
- Know names and positions of the Health and Wellness Center staff.
- Be treated with respect, consideration, privacy and dignity.
- Receive accurate information about your care in understandable terms.
- Informed consent.
- Participate in decisions regarding your care.
- Review your medical records with Health and Wellness Services staff.
- Be informed of Health and Wellness Center services and related costs.
- Refuse treatment.
- Appropriate referrals to other providers and services.
You have a responsibility to:
- Comply with all regulations regarding immunization, physical exam, and health insurance requirements.
- Be able to provide accurate ID information if requested.
- Seek medical attention promptly.
- Provide accurate information about you and your condition.
- Ask for clarification about anything you do not understand.
- Arrive on time for appointments or cancel in a timely manner.
- Report any significant changes in symptoms or failure to improve.
- Be involved in the development and comply with your treatment.
- Seek non-emergency care during regular hours.
- Act in a considerate and cooperative manner with the Health and Wellness Center staff and other clients.
- Provide useful feedback about services/policies to Health and Wellness Center Director or Advisory Committee.
- Return borrowed equipment in a timely manner.
